Home Visit Questionaire

To help us incorporate the home visits into the student’s overall program, and to reinforce to the student that all of the adults on his or her team are working together, NSI Academy has developed a home visit questionnaire that gives parents the opportunity to give us feedback about the visit. An example of that form is attached. This information is very important in our work with students. Please notice that there is a section for parents to list any new articles the student is bringing back to NSI Academy. With more than 100 students in our program, it is important to keep an accurate inventory of each student’s belongings. You will assist us in that effort by documenting new items your child brings back from a home visit.
